Social Care Program

We are here to support individuals with Medicaid in Wyoming County, NY with access to essential services. We are part of the Social Care Network through WeLinkCare by FLIPA, to help individuals with Medicaid with initial screening and navigation services. The Social Care services that Peers Together of Wyoming County provides include care management, transportation, housing assistance, nutrition and navigation to social care services.
